Community Health Worker

Job Description

As a Community Health Worker, you'll be pivotal in fostering health education and connecting individuals to healthcare resources within the community. You'll facilitate programs that address public health issues and collaborate with healthcare providers and organizations.

Responsibilities

• Conduct outreach and education efforts to promote public health initiatives.
• Provide one-on-one counseling and support to individuals and families.
• Create and facilitate community events and activities to promote healthy behaviors.
• Collect and analyze data related to public health outcomes and report findings to the team.
• Collaborate with other agencies and community organizations to coordinate services and resources.
• Assist in grant writing and budget management for public health projects.

Requirements

Education

• Bachelor's degree in Public Health or related field
• Master's degree in Public Health preferred

Experience

• 2+ years of experience in public health outreach
