Case Worker

Job Description

We are seeking a dedicated Case Worker to join our team. The successful candidate will be responsible for assessing client needs, developing case plans, and providing ongoing support to promote well-being and stability. Join us in making a positive impact in the community.

Responsibilities

• Assess client needs through interviews and questionnaires.
• Create and maintain comprehensive case files to track client progress.
• Network with local organizations to facilitate client access to resources.
• Deliver individual and group counseling sessions.
• Prepare reports outlining client’s progress and recommended actions.
• Stay updated on legal and ethical standards in social work.
• Conduct follow-up meetings to review and adjust case plans.
• Engage in outreach activities to educate the community about services offered.
• Participate in multidisciplinary team meetings to coordinate care.
• Document all client interactions and interventions in a timely manner.

Requirements

Education

• Bachelor's degree in Social Work or related field
• Master's degree in Social Work preferred

Experience

• At least 2 years of experience in social services or similar

• Crisis Intervention Techniques
• Community Resource Networking

Soft Skills

• Problem Solving
• Team Collaboration

Certifications

• Certified Social Worker (CSW)
• CPR and First Aid Certification

Languages

• English: Fluent
• Mandarin: Conversational
