Responsibilities
Provide personal care and support to patients in their home environment.
Assist with daily tasks including mobility, meal preparation, and personal hygiene.
Encourage and support patients in maintaining their independence as much as possible.
Document changes in patient condition and communicate them to the healthcare team promptly.
Participate in care planning meetings as required.
Implement safety measures to prevent accidents and ensure patient comfort.
Offer companionship and engage patients in conversation and activities.

Be available for on-call shifts as needed to provide continuous care.

Requirements
Education
High School Diploma or equivalent
CNA or HHA certification preferred
Experience
1-2 years of experience in home health aid or caregiving roles
