Job Description

We are seeking a dedicated Pharmacist to join our team in Philadelphia to provide excellent pharmaceutical care. You will be responsible for assessing patient medication needs, providing consultations, and ensuring the safe dispensing of medications.

Responsibilities

  • Provide exceptional pharmaceutical care to all patients, ensuring optimal health outcomes.
  • Perform medication reconciliations for hospitalized patients transitioning to outpatient care.
  • Address patient inquiries regarding medications and therapy adjustments.
  • Maintain accurate medical records and complete documentation.
  • Collaborate with physicians to streamline medication orders.
  • Guide pharmacy technicians in their daily operations and duties.
  • Staying informed on new medications and therapeutic protocols.
  • Implement strategies to improve the efficiency of pharmacy services.
  • Contribute to policy development and quality improvement initiatives.
  • Provide recommendations for formulary management and alternative medication choices.

Requirements

Education
  • Pharm.D. or equivalent
Experience
  • 3-5 years of experience in clinical pharmacy or retail settings
