Job Description

We are seeking a dedicated Nurse Practitioner to join our team in Jacksonville. This role requires a committed individual focused on providing exceptional patient care through assessment, diagnosis, and treatment planning.

Responsibilities

  • Assess, diagnose, and treat patients with various health conditions.
  • Collaborate effectively with a healthcare team to deliver coordinated care.
  • Prescribe and manage pharmacological and non-pharmacological interventions.
  • Document patient encounters accurately and thoroughly.
  • Participate in case discussions and clinical reviews to improve care quality.
  • Educate patients about disease prevention and health maintenance techniques.
  • Lead initiatives to enhance patient education resources.
  • Conduct follow-up evaluations to monitor patient progress.
  • Foster relationships with community organizations to enhance health services availability.
  • Maintain professional development through continuous education and training.

Requirements

Education
  • Master's degree in Nursing or related field
  • Certification as Family Nurse Practitioner is preferred
Experience
  • 3+ years of healthcare experience with direct patient care
