Job Description
We are seeking a dedicated Nurse Practitioner to join our team in Jacksonville. This role requires a committed individual focused on providing exceptional patient care through assessment, diagnosis, and treatment planning.
Responsibilities
Assess, diagnose, and treat patients with various health conditions.
Collaborate effectively with a healthcare team to deliver coordinated care.
Prescribe and manage pharmacological and non-pharmacological interventions.
Document patient encounters accurately and thoroughly.
Participate in case discussions and clinical reviews to improve care quality.
Educate patients about disease prevention and health maintenance techniques.
Lead initiatives to enhance patient education resources.

Maintain professional development through continuous education and training.
Requirements
Education
Master's degree in Nursing or related field
Certification as Family Nurse Practitioner is preferred
Experience
3+ years of healthcare experience with direct patient care
