We are seeking a dedicated Pharmacist to join our pharmacy team in a patient-centered environment. The ideal candidate will be responsible for dispensing medications, providing patient care, and ensuring compliance with all regulations.

Responsibilities

• Evaluate and document patient symptoms and conditions.
• Develop and implement individualized treatment plans.
• Utilize therapeutic modalities to promote healing.
• Educate patients about exercise techniques and lifestyle changes.
• Collaborate with interdisciplinary teams to optimize patient care.
• Monitor and document patient progress and outcomes.
• Assist in designing and implementing wellness programs.
• Maintain appropriate medical documentation in compliance with regulations.

Requirements

Education

• DPT or related degree from an accredited institution
• Current state licensure required

Experience

• 2+ years of experience in direct patient care

• Rehabilitation Techniques

Certifications

• Licensed Physical Therapist
• Basic Life Support (BLS)

Languages

• English: Fluent
