Job Description

We are seeking a dedicated Pharmacist to join our healthcare team in Philadelphia, PA. The ideal candidate will be responsible for ensuring safe and effective medication use by cooperating with healthcare professionals and providing exceptional patient care.

Responsibilities

• Evaluate and manage potential medication therapy problems for patients.
• Educate patients and families about drug interactions and side effects.
• Coordinate with healthcare professionals to promote safe medication practices.
• Conduct regular health screenings and administer vaccines.
• Train and mentor pharmacy staff on best practices.
• Participate in quality improvement initiatives within the pharmacy.
• Respond to medication inquiries and adverse drug reactions promptly.
• Ensure the pharmacy complies with local, state, and federal regulations.

Requirements

Education

• Doctor of Pharmacy (Pharm.D.)
• Clinical pharmacy residency preferred

Experience

• 3-5 years of clinical pharmacy experience is preferred
