Job Description

As a Healthcare Social Worker, you'll play a pivotal role in supporting patients through their medical journey. You will assess social, emotional, and psychological needs while developing individualized care plans. Your work will involve coordinating services and advocating for patient rights, ensuring they have access to essential healthcare resources.

Responsibilities

  • Assess and identify patients' social and emotional needs during home visits.
  • Create personalized service plans that address the unique needs of each patient.
  • Facilitate access to community resources and assistance programs.
  • Deliver supportive counseling to help patients cope with health-related challenges.
  • Work closely with doctors, nurses, and therapists to coordinate patient care.
  • Monitor patient progress and adjust care plans based on real-time feedback.
  • Educate patients and families about available health services and resources.
  • Engage in community outreach programs to improve patient awareness and access.
  • Provide crisis intervention and conflict resolution strategies as needed.

Requirements

Education
  • Bachelor's degree in Social Work or related field
  • Master's degree in Social Work is preferred
Experience
  • Minimum of 2 years of experience in social work or healthcare setting
