Job Description

As a Case Manager at Community Health Solutions, you will play a vital role in coordinating care and supporting clients with diverse needs. Your responsibilities will include developing personalized care plans, collaborating with healthcare professionals, and ensuring clients receive the best services available.

Responsibilities

  • Evaluate clients' situations and recommend appropriate services.
  • Develop and document care plans tailored to individual client needs.
  • Establish strong relationships with clients and their families.
  • Ensure continuity of care through follow-ups and coordination of services.
  • Provide crisis intervention and support when necessary.
  • Educate clients on available resources and support services.
  • Collaborate with community organizations to enhance service delivery.
  • Prepare reports and updates regarding client status for management.
  • Engage in ongoing professional development and training sessions.

Requirements

Education
  • Bachelor's degree in a relevant field
  • Master's degree in Social Work or related field preferred
Experience
  • 2-4 years of experience in case management or a related healthcare role
