Job Description

We are seeking a dedicated Telehealth Case Manager to provide remote healthcare support to patients utilizing advanced telecommunication technologies. This role involves working closely with healthcare professionals, patients, and caregivers to ensure effective care delivery and management.

Responsibilities

  • Provide remote case management services through telehealth platforms.
  • Assess patient needs, developing individualized care plans with the healthcare team.
  • Coordinate referrals and appointment bookings for patients.
  • Educate patients on self-care techniques and empower engagement in treatment plans.
  • Communicate effectively with patients, caregivers, and healthcare professionals.
  • Ensure adherence to legal and ethical guidelines in delivering telehealth services.
  • Maintain detailed and up-to-date patient documentation and records.
  • Stay informed about telehealth best practices and new technologies.
  • Participate in continuous quality improvement activities and training sessions.

Requirements

Education
  • Bachelor's degree in Nursing, Healthcare Management, or related field
  • Master's degree is preferred
Experience
  • Minimum 2 years of experience in healthcare, case management, or telehealth.
