Job Description

We are seeking a dedicated Nurse Practitioner to join our esteemed healthcare team in Los Angeles. This role involves providing comprehensive nursing care to patients, performing assessments, diagnosing conditions, and developing treatment plans. The ideal candidate will have strong clinical skills and a passion for enhancing patient well-being.

Responsibilities

  • Perform health risk assessments and screenings.
  • Monitor and evaluate patient progress towards health goals.
  • Advocate for patient-centered care and improved health outcomes.
  • Engage in interdisciplinary collaboration to enhance care delivery.
  • Provide urgent care as needed within scope of practice.
  • Document clinical findings and care provided in electronic health records.
  • Stay informed about healthcare policies and regulations.
  • Conduct patient education workshops as needed.
  • Facilitate care transitions for discharged patients.
  • Participate in ongoing professional development and training.

Requirements

Education
  • Doctor of Nursing Practice (DNP) is preferred
  • Nurse Practitioner Certification (ANCC) is required
