Job Description

We are seeking a passionate Social Worker dedicated to enhancing the well-being of individuals and families in Jacksonville. In this role, you will engage with diverse clients, providing counseling, support, and resources to help them navigate life's challenges.

Responsibilities

  • Assess and diagnose clients’ emotional and social needs.
  • Develop and execute individualized treatment plans tailored to client goals.
  • Advocate for clients in accessing community services and benefits.
  • Coordinate multidisciplinary team meetings to discuss client progress.
  • Educate clients and their families about coping strategies and available resources.
  • Engage in community networking to foster new partnerships.
  • Maintain compliance with ethical and legal standards in case management.
  • Conduct follow-up evaluations to measure client satisfaction and outcomes.

Requirements

Education
  • Bachelor's degree in Social Work (BSW)
  • Master's degree in Social Work (MSW) preferred
Experience
  • 3-5 years of experience in social services or a mental health setting
