Pharmacist

Job Description

We are seeking a dedicated Pharmacist to join our Tampa team. The ideal candidate will be responsible for providing patient-centered pharmacy services, including medication management, counseling, and collaboration with healthcare professionals. In this role, you will ensure the safe and effective use of medications, while also playing a key role in educating patients about their therapies.

Responsibilities

• Evaluate patient medication therapy outcomes.
• Provide recommendations for medication-related problems.
• Develop and maintain strong patient-pharmacist relationships.
• Assist in formulary management and medication procurement.
• Participate in multidisciplinary team meetings.
• Provide clinical pharmacy services to targeted populations.
• Ensure proper documentation in patient records.
• Stay current on new drug therapies and advancements in pharmacy.

Requirements

Education

• Doctor of Pharmacy (PharmD)

Experience

• 2+ years of clinical pharmacy experience preferred

Technical Skills

• Patient Counseling
• Drug Utilization Review

Certifications

• Pharmacy Licensure
• Certified Geriatric Pharmacist (CGP) - Optional

Languages

• English: Fluent

Advantageous
