Job Description

Join our team as a Licensed Practical Nurse (LPN), where you will be responsible for delivering essential nursing care and support to patients. Your role will include monitoring vitals and collaborating closely with healthcare professionals to enhance patient outcomes.

Responsibilities

• Deliver nursing care based on the established care plan for each patient.
• Act as a liaison between patients, families, and healthcare providers.
• Document patient assessments and care activities in the electronic health record.
• Ensure a safe and clean environment for patients.
• Follow infection control protocols and safety guidelines.
• Assist with the onboarding and orientation of new nursing staff.
• Participate in continuous education and training programs.

Requirements

Education

• High school diploma or equivalent
• Completion of an accredited LVN program

Experience

• Experience in a long-term care facility is a plus but not mandatory.
