Job Description

The Healthcare Social Worker will provide social and emotional support to patients and their families, ensuring they navigate the healthcare system effectively. This role involves developing care plans and coordinating services to address the unique needs of each individual.

Responsibilities

• Conduct initial assessments to identify patients' social, emotional, and financial needs.
• Facilitate access to community resources and support services.
• Ensure seamless transitions of care among different healthcare providers.
• Provide emotional support to individuals and families coping with health challenges.
• Develop and maintain collaborative relationships with patients and their families.
• Utilize electronic health records (EHR) systems for documentation.
• Educate healthcare teams about the social aspects of patient care.
• Monitor and manage caseloads effectively to meet patient needs.
• Develop workshops and support groups for patients and families.
• Stay updated on healthcare policies affecting social work practice.

Requirements

Education

• Master's degree in Social Work (MSW)
• Certification in healthcare social work (CHSW) preferred

Experience

• At least 4 years of experience in healthcare settings

Languages
