Job Description

We are looking for a highly skilled and experienced Primary Care Nurse Practitioner (PCNP) who is passionate about providing exceptional patient-centered care. This position will involve collaborating with physicians, nurses, and other healthcare professionals while maintaining a commitment to quality and excellence.

Responsibilities

  • Perform comprehensive evaluations and manage health concerns.
  • Prescribe appropriate medications and therapies.
  • Educate patients on chronic disease management and preventive care.
  • Coordinate care with specialists and community resources.
  • Document all patient encounters accurately and in a timely manner.
  • Engage in telehealth and virtual care where appropriate.
  • Contribute to team meetings and case discussions to ensure collaborative care.
  • Mentor and provide guidance to junior nursing staff.

Requirements

Education
  • Master's degree in Nursing
  • Certification as a Family Nurse Practitioner (FNP) preferred
Experience
  • 5+ years of experience in primary care settings preferred.
