Job Description
We are looking for a highly skilled and experienced Primary Care Nurse Practitioner (PCNP) who is passionate about providing exceptional patient-centered care. This position will involve collaborating with physicians, nurses, and other healthcare professionals while maintaining a commitment to quality and excellence.
Responsibilities
- Perform comprehensive evaluations and manage health concerns.
- Prescribe appropriate medications and therapies.
- Educate patients on chronic disease management and preventive care.
- Coordinate care with specialists and community resources.
- Document all patient encounters accurately and in a timely manner.
- Engage in telehealth and virtual care where appropriate.
- Contribute to team meetings and case discussions to ensure collaborative care.
- Mentor and provide guidance to junior nursing staff.
Requirements
Education
- Master's degree in Nursing
- Certification as a Family Nurse Practitioner (FNP) preferred
Experience
- 5+ years of experience in primary care settings preferred.
