Community Health Services is seeking compassionate and dedicated Healthcare Social Workers to provide support to individuals and families navigating health-related challenges. You will play a vital role in assessing needs, developing care plans, and connecting clients to resources.

Responsibilities

• Assess client needs and create tailored treatment plans addressing social and emotional health.
• Manage cases by following up with clients, tracking progress, and adjusting plans as necessary.
• Educate clients and their families about health conditions and available community resources.
• Collaborate with multidisciplinary teams to integrate services for comprehensive client care.
• Participate in staff meetings and training to promote best practices in social work.

Requirements

Education

• Master's degree in Social Work (MSW)

Experience

• 2+ years of experience in social work or healthcare setting
