Job Description

As a Community Health Worker, you will play a crucial role in connecting individuals and families to necessary resources. Your responsibilities will include conducting outreach, providing education, and advocating for community members to ensure they receive the support they need.

Responsibilities

• Identify and connect individuals with relevant community resources.
• Coordinate outreach events to promote health education and services.
• Develop educational materials and distribute them to community members.
• Assist families in accessing healthcare services and benefits.
• Maintain a strong knowledge of community resources and updates.
• Participate in training and development opportunities.
• Foster partnerships with local agencies and organizations.
• Track and analyze outreach efforts to improve service delivery.

Requirements

Education

• Bachelor's degree in social work, public health, or related field

Experience

• Minimum of 2 years of experience in community outreach or social services

Technical Skills

• Microsoft Office Suite

Certifications

• CPR and First Aid Certification

Languages

• English: Fluent
• Spanish: Preferred

Advantageous
