Job Description

We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team in Philadelphia. In this role, you will be responsible for assessing patient medication needs, providing patient care, and ensuring safety and compliance in medication dispensing.

Responsibilities

  • Assess patient medication needs and contraindications.
  • Collaborate with doctors to customize medication therapies.
  • Provide clinical pharmacy services including medication therapy management.
  • Conduct patient medication reviews and monitor adherence.
  • Participate in community health initiatives and outreach programs.
  • Ensure proper storage and handling of medications.
  • Train and mentor pharmacy interns and staff.
  • Participate in continuing education and professional development activities.

Requirements

Education
  • Doctor of Pharmacy (Pharm.D.) degree
  • Pharmacy residency or fellowship is preferred
Experience
  • 3+ years of experience in clinical pharmacy
