Job Description

We are seeking a dedicated Chronic Condition Nurse to join our healthcare team. You will be responsible for implementing care strategies for patients with chronic conditions like diabetes and heart disease, providing education and support to both patients and their families.

Responsibilities

  • Implement and evaluate chronic disease management protocols.
  • Coordinate care across multiple healthcare providers.
  • Assist in developing community outreach initiatives for chronic disease education.
  • Conduct patient education sessions on lifestyle modifications.
  • Lead support groups for patients living with chronic conditions.
  • Utilize health technology to monitor patient progress.
  • Facilitate communication between patients and healthcare providers.
  • Ensure compliance with healthcare regulations and standards.

Requirements

Education
  • Bachelor of Science in Nursing (BSN) strongly preferred
  • Master's degree in Nursing or relevant field is a plus
Experience
  • 3-5 years of experience in nursing with a focus on chronic conditions.
Technical Skills
  • Patient education skills
  • Ability to utilize patient management software
Soft Skills
  • Analytical thinking
  • Collaboration
Certifications
  • Basic Life Support (BLS) certification required
  • Certified Diabetes Educator (CDE) preferred
Languages
