Job Description

Join our dedicated team as a Mental Health and Substance Abuse Social Worker. You will play a vital role in providing support to clients with various mental health challenges and substance use issues. Your expertise will be essential in developing individualized care plans and connecting clients with necessary resources.

Responsibilities

  • Conduct intake assessments and develop treatment plans for clients.
  • Facilitate group and individual counseling sessions.
  • Work closely with community partners to ensure comprehensive care for clients.
  • Maintain complete and up-to-date documentation and case notes.
  • Participate in staff meetings and case reviews to enhance service delivery.
  • Engage in community outreach and resource development activities.
  • Provide crisis intervention and support as needed to clients and families.
  • Assist clients in navigating complex social service systems.
  • Maintain confidentiality and professional boundaries in all interactions.
  • Continuously assess and improve care strategies based on client feedback.

Requirements

Education
  • Bachelor's degree in Social Work or related field
  • Master's degree in Social Work is highly desirable
Experience
  • 3+ years of experience working with mental health and substance abuse populations
