Job Description

As a Certified Health Care Social Worker, you will play a crucial role in supporting patients and their families through challenging healthcare journeys. You'll work collaboratively with physicians, nurses, and case managers to provide emotional support and address social needs, helping to improve patient outcomes and overall quality of life.

Responsibilities

• Assess patients' psychosocial needs through interviews and reviews of medical records.
• Collaborate with other healthcare professionals to ensure effective delivery of care.
• Provide counseling to help patients cope with emotional challenges related to their health conditions.
• Advocate for services and resources that address patient and family needs.
• Facilitate group sessions and workshops focused on health education and support.
• Maintain timely and comprehensive documentation of all interactions and interventions.
• Engage in ongoing training and professional development to stay current with best practices.

Requirements

Education

• Master's degree in Social Work from an accredited program
• Certification as a Certified Health Care Social Worker (CHCSW)

Experience

• 3+ years of experience in social work within healthcare settings preferred.

Certifications
