Job Description

We are seeking a compassionate Healthcare Social Worker to join our team. In this role, you will assess clients' needs and create tailored care plans to support their emotional and social well-being. You will act as a vital link between clients and community resources, ensuring they receive the assistance they need.

Responsibilities

• Conduct client intakes and assessments to identify needs.
• Create and implement comprehensive care plans tailored to clients.
• Facilitate access to healthcare and social services.
• Document and report on clients' status and treatment outcomes.
• Act as a liaison between different social service agencies.
• Attend training workshops for professional development.
• Provide emotional and practical support to clients in crisis situations.
• Engage in advocacy efforts for policy changes benefiting clients.
• Participate in community outreach programs.
• Conduct follow-up visits to assess ongoing client needs.

Requirements

Education

• Bachelor’s degree in Social Work or Psychology
• Master’s degree preferred but not required

Experience

• At least 2 years of experience in healthcare or social services

• Data Management Software

Certifications

• Certified Social Worker (CSW) or similar

Languages

• English: Fluent
• Spanish: Conversational
