Job Description

We are looking for an experienced Social Worker to join our compassionate team. You will be responsible for providing direct support to individuals and families, conducting assessments, and developing treatment plans tailored to their unique needs.

Responsibilities

• Conduct home visits and community outreach to engage clients.
• Support families in navigating social services and benefits.
• Document all interactions, referrals, and progress toward treatment goals.
• Attend team meetings and contribute to collaborative care strategies.
• Monitor clients' progress and adjust care plans as needed.

Requirements

Education

• Master's degree in Social Work (MSW)

Experience

• 3+ years of social work experience preferred

• Behavioral Assessment

Certifications

• Licensed Clinical Social Worker (LCSW)

Languages

• English: Fluent

Advantageous

• Bilingual in English and Spanish: Ability to communicate effectively with Spanish-speaking clients.
• Experience in group therapy: Background in facilitating group therapy sessions.
