Community Health Worker

Job Description

As a Community Health Worker, your primary role will be to assess the needs of individuals facing behavioral health issues. You will develop personalized care plans, educate clients about their conditions, and ensure they can access the services they need to thrive.

Responsibilities

• Conduct needs assessments to identify barriers to care.
• Facilitate referrals to mental health services and other community resources.
• Develop resources and workshops for community education on behavioral health.
• Support clients in navigating healthcare systems and insurance processes.
• Track client progress and modify care plans as needed.
• Engage in community outreach to foster relationships with local organizations.
• Collaborate with multidisciplinary teams for comprehensive care delivery.
• Provide crisis intervention when necessary.

Requirements

Education

• Master's degree in Behavioral Health or related field

Experience

• 3+ years of experience in behavioral health is preferred.

Technical Skills

• Data analysis software (e.g., SPSS, SAS)
• Telehealth platforms

Soft Skills

• Team collaboration
• Crisis intervention skills

Certifications

• CPR and First Aid Certification

Languages

• English: Fluent
• Spanish: Fluent
